Crave Health is a preferred provider for Premera Insurance. Premera will cover 4 sessions with an RD per year with one of the following diagnoses:

Individual Nutrition Counseling
Initial Consultation: 20 mins
Don't know where to start? Come in for a consultation to discuss your goals and desires for your health. Using the information from the consultation we will be able to take the next step to decide if you need a 50 min assessment, just several follow up sessions or a different plan of action. The $20 consultation fee will be waived with the purchase of any service package day of consultation. $20

Basic Nutrition Assessment: 50 min
A first time health and nutrition interview where the client and dietitian discuss personal and family health history, weight changes, eating habits, current struggles and health goals. From there we can begin to determine the best course of care for you. It includes baseline measurements of weight, height, waist circumference, blood pressure, glucose and heart rate. $100
Nutrition Follow Up Session: 30 mins
Nutrition Follow up sessions are designed to follow progress and make any adjustments to the treatment plan. These are also important for problem solving and teaming together to design the most individualized care possible. $50
Professional 3-Day Dietary Analysis
Using professional nutritional analysis software we can analyze a 3-day food record. Comes complete with colorful print out showing vitamin and mineral content, macronutrient breakdown, where you are in comparison to your recommended daily values and areas for improvement. $40
Group Nutrition Counseling
Disordered Eating Support Group – Wednesdays 8-9pm
For those struggling with emotional overeating, restricting, binging, purging or other disordered eating habits. This meeting is specifically for adults.

The Lord’s Table Weight Loss Group – “Build Your Own Group”
This is a build your own group for 4-8 people. Perfect for a group of men or women that want to lose weight in a structured program with the support of friends. Meets once per week for one hour at a time chosen by the group members and dietitian. Each person receives an individualized meal plan and tailored nutritional recommendations. Any additional follow-up sessions are discounted. $300 per person
Statement of Faith: Ashley is a Christian, but does not incorporate her own personal faith or beliefs into her work at Crave Health unless specifically requested by the client. Unless specifically stated, like the group above, treatment and sessions will not integrate aspects of faith.
Sliding Scale Fees
Please see income chart to determine your sliding scale fee for the session of your choice. Sliding scale fees are offered for those in financial hardship, and must show proof of income at time of service.
